① Print out the attached "Application for Provisional Degree Certificate"

② Fill in all the blanks of "Application for Provisional Degree Certificate" and submit it to Section of Academic and Student Affairs(#202 Admin. Building)

③ You can print out the Provisional Degree Certificate from the certificate machine on the 1st floor of Admin. Building after check of the staff
